Melanie is a experienced clinician with a background in paediatric dentistry and orthodontics. Her professional journey began with Vocational Training (VT) in General Dental Practice, followed by a role as a Dental Officer in the Community Dental Service (CDS). Having developed a passion for providing children with the best experience and care possible she elected to further develop her clinical expertise as a House Officer in Paediatric Dentistry at Guy’s Hospital and as a Clinical Assistant in Orthodontics at SMH.
Most recently, she served as a Senior Dental Officer (SDO) in Paediatric Dentistry and Orthodontics at the Berkshire Healthcare Trust and as a Dentist with a Special Interest in Orthodontics within a specialist orthodontic practice. This extensive career across community, hospital, and specialist settings treating both paediatric and orthodontic patients has provided her with a unique insight into how best to manage complex cases involving compromised first permanent molars.

Management of the compromised first permanent molar in children
This presentation provides a comprehensive guide for dental professionals on managing compromised first permanent molars (cFPM) in children. It highlights that up to 25% of UK children are affected by cFPM, primarily due to dental caries and Molar Incisor Hypomineralisation (MIH). The primary objective is to equip clinicians with the knowledge and confidence to make timely and appropriate treatment decisions, thereby reducing unnecessary orthodontic referrals and improving patient outcomes.
Critical factors that can influence treatment choices are outlined, including restorability, patient cooperation, parental preference and the patient’s age and stage of dental development to allow for interceptive benefits. It discusses the rationale behind the Thames Valley and RCS guidelines, which generally advocate for restoring viable teeth while extracting those with a poor prognosis. Furthermore, the presentation details how different malocclusions may play a part in treatment planning for the cFPM. It highlights the benefits of knowing how to use the Index of Orthodontic Treatment Need (IOTN) as a tool for determining NHS treatment eligibility and obtaining informed consent. Finally, it discusses the relevance of regional variations in service provision when patients are referred for treatment in secondary care. Providing the Community dental services across Berkshire, Oxfordshire and Buckinghamshire regions as an example.
