Dr. Anibal Diogenes received his dental degree from UFPE in Brazil, his M.S. in Molecular Biology from the University of Nebraska, and his Ph.D. in Pharmacology and his Certificate in Endodontics from the University of Texas Health Science Center at San Antonio. Dr. Diogenes is the Chair of the Department of Endodontics at the University of Texas Health at San Antonio, and a Director of the American Board of Endodontics. His areas of research include inflammation, pain, and regenerative endodontics.  He has received the International Association for Dental Research (IADR) Distinguished Research Award, the AAE Osetek Award, and the Minnie Stevens Piper Foundation Professorship Award. Dr. Diogenes maintains a clinical practice limited to Endodontics in San Antonio, Texas.

Synopsis

Dental pulp  and apical tissues are highly innervated, making pain difficult to manage. This lecture reviews trigeminal nociception, inflammatory neuronal changes, and anesthetic/analgesic pharmacology with a clinical focus on optimizing intra- and postoperative pain control in endodontics

At the conclusion of this lecture, participants will be able to:

  1. Describe the major components of the trigeminal nociceptive pathway.
  2. Explain the mechanism of action of local anesthetics and the primary reasons for anesthesia failure.
  3. Identify the pain mechanisms responsible for inadequate postoperative pain relief.
  4. Discuss the mechanisms of action, indications, and contraindications of major analgesic drug classes.
  5. Evaluate the therapeutic value of non-NSAID agents in the management of endodontic pain.

Adequate pain management requires a thorough understanding of these changes is critical for clinicians seeking to deliver effective care. Local anesthesia remains a cornerstone of dental treatment. Similarly, the selection and timing of analgesics must be optimized to enhance efficacy while minimizing adverse effects. This lecture aims to 1) review the trigeminal nociceptive system and its modulation ; 2) review  the pharmacology and therapeutic use of local anesthetics and analgesics, highlighting evidence-based strategies for improving patient comfort.
